When mental health concerns begin to interfere with your ability to work, you may need additional support and documentation for a leave of absence or workplace accommodations. At Tranquil Seas Therapy, we can help guide you through the therapeutic and clinical documentation process when FMLA or other medical certification paperwork may be appropriate.
Our goal is to provide thoughtful, clinically appropriate documentation while continuing to focus on your mental health and treatment needs.
How We Can Help
Your therapist may work with you to:
Evaluate how your mental health symptoms are affecting your daily functioning and ability to work
Develop and continue an appropriate treatment plan
Monitor your symptoms and progress throughout treatment
Determine whether available clinical information supports the need for FMLA or other medical documentation
Complete clinically appropriate documentation when requirements have been met
Provide updates or revisions when clinically indicated
Our Therapeutic Process
To ensure that documentation is based on an adequate understanding of your clinical needs, a minimum of three therapy sessions is required before FMLA paperwork will be considered. Additional sessions may be necessary depending on your individual circumstances.
Therapy is generally expected to occur on a weekly or biweekly basis, unless your therapist clinically recommends a different frequency. Ongoing participation allows your therapist to appropriately assess your symptoms, functioning, treatment progress, and continued needs.
Ongoing Documentation
FMLA and related documentation may require updates over time. Paperwork may need to be reviewed or updated every 30 days or as clinically indicated.
Continued participation in treatment is important. Missed appointments, repeated cancellations, or failure to participate in recommended treatment may delay or prevent the completion or continuation of documentation.

Information included in paperwork will be based on your clinical record, treatment progress, functional impairment, and professional judgment.

Documentation will only include information that is clinically accurate and supported by the medical record.
Confidential
Evaluations and documentation become part of your confidential clinical record and are maintained in accordance with HIPAA and applicable laws.
Collaborative
We will work with you throughout the therapeutic process and communicate what information or documentation is needed.
What We Need From You
To help the process move smoothly, clients are responsible for:
Completing any portions of the forms that are designated for you
Providing necessary releases of information
Providing supporting documentation when applicable
Attending therapy as recommended
Allowing adequate time for paperwork to be completed
Communicating with your therapist about changes in your symptoms, functioning, or work-related needs
Paperwork Fees & Timeline
Therapy sessions may be billed through your insurance when applicable. Administrative paperwork services are separate from therapy and are not billed to insurance.
FMLA/Initial Paperwork: $120
Additional paperwork, revisions, or communications: $30 per 15-minute increment
Payment is due before paperwork is released, and administrative fees are non-refundable once work has begun.
Paperwork is generally completed within 7–10 business days after all clinical requirements, completed forms, and payment have been received.
